Klumpke’s Palsy: A Less Common but Serious Birth Injury

Klumpke’s palsy is a relatively uncommon form of neonatal brachial plexus injury that can affect a baby’s hand, wrist, and forearm. Unlike the more familiar Erb’s palsy, which primarily affects the upper portion of the brachial plexus, Klumpke’s palsy involves the lower portion, generally the C8 and T1 nerve roots. What Is Klumpke’s Palsy?

Klumpke’s palsy is an injury involving the lower trunk of the brachial plexus, typically affecting the C8 and T1 nerve roots. The brachial plexus is a network of nerves extending from the spinal cord through the neck and shoulder and into the arm and hand. These nerves control movement and sensation throughout the upper extremity. The location of the injury distinguishes Klumpke’s palsy from Erb’s palsy. Erb’s palsy primarily affects the upper brachial plexus, particularly C5 and C6, and commonly produces weakness involving the shoulder and upper arm. Not every newborn with weakness or reduced movement of the arm has a brachial plexus injury, and physicians may need to distinguish Klumpke’s palsy from conditions such as a fracture or other neurological problems.

How Can Klumpke’s Palsy Occur During Childbirth?

The traditional mechanism associated with Klumpke’s palsy is excessive traction on the lower brachial plexus, particularly a force that produces significant abduction of the baby’s arm. Some medical literature has associated the injury with situations in which an infant’s arm becomes positioned above the head during delivery. Breech presentation has also been reported in clinical series involving Klumpke’s palsy.

The precise mechanism must be evaluated on a case-by-case basis. A brachial plexus injury occurring during childbirth does not automatically establish medical malpractice. What Are the Symptoms of Klumpke’s Palsy?

Because the lower brachial plexus controls important functions involving the forearm and hand, Klumpke’s palsy can produce weakness or paralysis in those areas. An affected newborn may have difficulty moving the wrist or fingers. Weakness of the intrinsic muscles of the hand can produce an abnormal hand posture sometimes described as a “claw hand” deformity. The presence of Horner syndrome in an infant with a brachial plexus injury can indicate more extensive neurological involvement and may be relevant when evaluating the severity and prognosis of the injury.

How Is Klumpke’s Palsy Diagnosed?

Diagnosis generally begins with a physical and neurological examination of the newborn. Doctors may evaluate movement of the shoulder, elbow, wrist, and fingers; muscle strength; reflexes; and sensory responses. The child’s progress can then be monitored through repeated examinations. Additional testing may be appropriate depending on the circumstances. Medical professionals may use imaging or electrodiagnostic studies to help characterize the injury and distinguish it from other conditions that can cause decreased movement of an extremity. A fracture of the clavicle or humerus, for example, can cause an infant to avoid moving an arm and may initially resemble a nerve injury.

Can Klumpke’s Palsy Heal on Its Own?

What Treatment Is Available for Klumpke’s Palsy?

Treatment depends on the child’s symptoms and the severity of the nerve damage. Physical and occupational therapy may be used to maintain mobility, encourage appropriate motor development, and reduce complications associated with weakness or abnormal positioning. Children with significant or persistent deficits may be evaluated by specialists in pediatric nerve and upper-extremity injuries. In some severe cases, surgical procedures may be considered to repair or reconstruct damaged nerves. The appropriate timing and type of intervention depend on the specific injury and the child’s progress. Early evaluation can be important because persistent weakness may affect the development and function of the hand and upper extremity.

What Are the Long-Term Effects of Klumpke’s Palsy?

The long-term effects can vary considerably. A child with a mild injury may experience substantial recovery. A more severe injury, however, can result in persistent weakness, limited hand function, sensory abnormalities, muscle imbalance, or difficulty performing fine motor activities. These limitations can affect everyday activities as the child grows. Tasks involving writing, manipulating objects, dressing, using tools, participating in sports, or performing other activities requiring fine motor control may become more difficult. A permanent impairment can also affect educational opportunities, future employment, independence, and quality of life.

Is Klumpke’s Palsy Always Caused by Medical Malpractice?

The presence of Klumpke’s palsy after delivery does not by itself establish that a doctor, nurse, hospital, or other healthcare provider was negligent. Neonatal brachial plexus injuries can occur in circumstances where there was no negligent conduct. At the same time, a birth injury warrants careful evaluation when the circumstances suggest that excessive force, improper management of a complicated delivery, or another preventable event may have contributed to the nerve damage.
